Kansas Lifeline Program — a service for
low-income customers
Customers meeting the eligibility criteria must
complete the
Lifeline* form and return it
to Golden Belt Telephone with proof of
participation in one of the programs.
Customers qualifying by income level must complete the
Under Income Level* form and return
it to Golden Belt Telephone with documentation of
annual household income. Acceptable
documentation includes the prior year's tax
return, three consecutive paycheck stubs, or a
retirement statement of benefits.
